How Do I....
Obtain A Repeat Prescription?
If you need regular prescriptions your doctor may not need to see you every time (this is by individual arrangement). There is no pharmacy here but we offer a range of options for getting your medication ready for you. Please make it clear to the receptionist if you would like your prescription sent to a local chemist and please request repeat prescriptions by bringing in or posting to us your computerised sheet and allow two working days. Please remember: to avoid errors, it is not possible to accept requests for repeat prescriptions over the telephone, unless it is quite impossible for you to come to the surgery.
If using the email facility below, please remember to allow two working days before trying to collect your prescription. In order for the prescription to be ready within this time it will need to be sent to us by 12 noon. If you email us after 12 noon you may need to allow another working day to be sure of your prescription being ready.
For efficiency, if we have a query about your prescription we will try to contact you on the phone number you give below. Please note we do not leave messages on answermachines.

How Do I....
Obtain Sickness Certificates?
For employees, doctors’ NHS certificates are not necessary for the first week of sickness. For the
first one to three days of sickness no certificate is required. If you are off for four
to seven days (including weekends) you need to complete an SC1 if you are self-employed, or an SC2
which is available from your employer. We also hold a small supply of SC2s at the health centre.
If you require a sick note from your doctor for insurance purposes, etc (or for your employer), please ask
the doctor for a private sick note for which a minimum fee of £15.20 is payable. This should be refunded by
your employer, if requested by them. If you are still sick after seven days (including weekends) ask your
doctor for a free NHS sick note.
